We help individuals and families across New York navigate the NHTD and TBI Waiver Programs so they can live safely in their preferred community setting rather than in a nursing home. Through Service Coordination, we work alongside participants, families, and providers to arrange services, manage care needs, and make sure the right supports are in place.
